Sub : Issue of Individual / Plastic Cards beneficiaries – reg.

            A copy of Circular received from CGHS Bangalore vide their letter No.Plastic Cards / CGHS/Admn/2521 dtd 7tg June 2012 regarding Issue of Individual / Plastic cards in enclosed.  A copy of guidelines on issue of individual plastic cards to each CGHS Beneficiary (serving and Delhi is also enclosed.

            The procedures to be followed in CGHS Bangalore are as follows :

1.    The new procedure shall be effective from 1st Aug 2012

2.    All blank CGHS cards issued to Central Government Departments should be surrendered to Main office positively before 31st July 2012 for onward transmission to the O/o the Addl Director, Kendriya Sadan, Koramangala.

4.    Heads of the PAO’s should take personal responsibility to ensure the correctness of the particulars furnished by the applicant before countersigning the requisition from ‘A’ as the sponsoring authority.  Contact Mobile Number or Landline number should Invariably be given by all CGHS beneficiaries in the form.

5.    Present Pay and Grade pay of serving employees should be mentioned separately.

6.    All central Government Department should mention the exact number of their employees covered under CGHS as o 31.7.2012.

Sub : Issue of Individual / Plastic Cards beneficiaries – reg.

            It has been decided to issue individual Plastic Card to all CGHS beneficiaries from 1st August 2012 by CGHS Koramangala, Bangalore, In this connection, the guidelines on issue of individual Plastic Cards to each CGHS beneficiary (serving and retired) are revised as follows:

1.    CGHS cards shall be issued only to the eligible central Government employees whose place of residence is situated within the coverage area of CGHS

2.    Requisition for CGHS shall be prepared in duplicate in Form “A”

3.    Requisitions for CGHS cards shall be accompanied by two copies of recent 3x5cm size individual photographs of all the family members of the government employee. One set of the photograph shall be signed on the back by the concerned beneficiary & enclosed with the application form.

4.    Requisitions shall be sent along with two copies of the challan as on Form ‘C’ duly filled in.

5.    On the occurrence of death, CGHS Cards issued to the government employee shall be withdrawn for cancellation.

6.    In case of change in entitlement for CGHS facilities, the Government  employee shall enclose the CGHS Card (s) with the application for issuing new card (s) with the revised entitlement.

7.    In case of mutilation, the mutilated card shall be enclosed with the application alongwith the challan in token of payment of the prescribed charges for issuing a new card.

8.    Duplicate cards may be issued on payment of prescribed fee with details of lost / misplaced cards.,

9.    The plastic cards issued by CGHS shall be valid for a period of five years from the date of issue.  The validity period shall also be indicated on the card.

All offices may strictly adhere to the laid down conditions given in the above circular.  The requisition from duly filled in by all officers & staff along with individual photographs may be forwarded to this office by 6th Aug 2012 for scrutiny and onward transmission to CGHS Bangalore.
